Appointments/Prescriptions

If you have a scheduled appointment with us, another physician has referred you to us. We address nephrology, chronic kidney disease, and internal medicine issues. We look forward to seeing you and encourage you to bring your written questions and medications with you. If you have a family member that you would like to have accompany you to your visit, we have no objections. We want to make your visit as comfortable as possible. Please try to be prompt for your appointment. Your appointment will be scheduled with the physician that is in the office on that day. This allows you to be familiar with each doctor and helps him to be informed regarding your conditions. If you must miss an appointment, let us know as soon as possible so that we may use the time to help someone else.

When your appointment is scheduled please prepare for the appointment by having all of your questions written down. This is also an excellent time to ask the doctor to write prescriptions that have expired. Please do not ask the physicians to write prescriptions that other practicioners have prescribed for you.

Please bring all of the medications and supplements that you are taking to your appointment each time you have an appointment.

If you run out of a prescription before your appointment and must call the office for a refill, we will need the following information:

  • Name of Medication
  • Strength (mg./mcg.)
  • Number of Tablets taken per day
  • Name & Phone number of the drug store that you use
  • Your DOB

Please do not call us to refill prescriptions written by other physicians.

Test Results:

If tests are ordered by the physician, it is your responsibility to call the office for results.

This will ensure that you receive your reports.
